Medical History & Immunization Record

All incoming freshmen, including those participating in Freshmen Connection, must complete the University of Maryland Medical History and Immunization Record. Failure to submit the completed health form to the University Health Center before the first day of the fall 2012 semester will result in a registration block on your student record, preventing you from registering for the spring 2013 semester.

Click here to access the printable Health Center form. Either mail the completed health form directly to the Health Center (the address is provided on the form) or turn it to the Health Center when you come to Freshmen Connection orientation.

Student Health Insurance Plan (SHIP)

Health insurance is mandatory for all newly admitted freshmen, including those participating in Freshmen Connection. All newly-admitted students must complete an on-line waiver at https://studentcenter.uhcsr.com/ to indicate whether they either:

  1. have insurance (i.e. through parent plans, family plans, or employer-sponsored plans) and wish to waive the requirement
    or
  2. wish to purchase SHIP available through the University of Maryland at http://www.firststudent.com

If a waiver is chosen, the website will require your name, date of birth, University ID#, e-mail, name of insurance company and insurance policy number. E-mail messages will automatically be generated to inform you of your waiver status upon submission of the waiver request.

If you do not waive out of the University Student Health Insurance Plan (SHIP) prior to the deadline (check the Health Center's website for exact deadline), you will automatically be enrolled in SHIP for the remainder of the 2012-2013 academic year and the non-refundable premium will be billed to your student account. Students who submit the waiver by the deadline will not be charged the University’s SHIP fee.
